Insert has always felt wrong to me too. However rather than replace
it with something else I would suggest removing it altogether.

I often ignore "insert" and use "edit" directly, which I think comes
from thinking of pass in terms of "filesystem" rather than
"database". Just like I always use the same vim command regard of
whether my file exists or not.
