[pass] [patch] fix lastpass import

Attached a new script:
- bash rather than ruby based
- supports "," inside password with help of csvtool
- permits specifying a password-store base directory to store imported passwords into
- dry-run + force modes
- can store based on the URL (optionally cleaned-up)

other output filename patterns (derived from lastpass fields) may be
added latter if really needed.

Tested successfully from lastpass export output
