[PATCH] make keepass2csv2pass.py more flexible

Hello,

I just migrated to pass from KeePassX and there were some options I wanted from
keepass2csv2pass.py that weren’t there. I cleaned it up a bit and added some
command line options. See the commit message for details.

This being my first patch, please let me know if I missed anything in the
contribution process!
